Transform a Paper / Word / Excel form to a digital Power App

ARO will deliver a form replacement app utilising their prebuilt solution and the form provided by the customer.

This will include: A Model Driven app for users and admins, accessible via web browser or PowerApps for mobile Two security roles, ‘Forms User’ and ‘Forms Admin’

• Users will have the restricted ability to: • Submit new forms • Read their previous submissions
• Admins will have the ability to also: • Deactivate records • Read all records • Export data to Excel • Set up new users and admins The App will provide:
• An ‘Excel like’ view of form entries displayed newest to oldest creation date
• Up to 30 questions or data points: • Free text • Date and Time • Currency Values • Yes/No Value • Option Sets • Multi Select Option Sets • Whole or decimal numbers • Phone Number • Email Address
• One digital signature capture
• Tracking of form statuses through; New, Completed, Inactive
• A notification by email when a form is either created or completed

This app can then be shared with users, and deployed to Teams or SharePoint site

The app will utilise Dataverse for storage, other detail and limitations are detailed within the associated statement of works.
