FormMachines supports flexible form hosting. You can host your form in your team or tenant, where users must login to complete a form. Or you can host your form openly, where anonymous users can fill out a form, the data can be accessed in Microsoft Teams.
This product is packaged as a Microsoft Teams App, it is easy to install and most updates are automatic.
