Detailed Description: The services start with a Discovery Workshop, designed to provide a roadmap to unlock high-value opportunities and leverage Microsoft Technologies including Microsoft Teams, Power Platform, Power BI, and Azure Data services. Discharge Planning provides a collaborative interface to improve communication, identify bottlenecks and help patients leave the hospital as quickly as clinically feasible. The cost varies base on the scope of the engagment. Discharge Planning can help Providers improve care and build the foundation for a larger digital health and patient care evolution. • Track progress in a unified patient view: See relevant information for every patient at a glance and easily view what tasks have been completed. • Capture even informal data: Make sure non-EHR info is being tracked, as care teams currently often track with paper notes, spreadsheets and word of mouth. • Communicate in context: Send messages and notifications in real-time and collaborate in secure one-to-one or one-to-many conversations, from any device. The workshop is typically followed by Design, Build and Implementation readiness phases based on the agreed direction with the client.
