Hitachi Solutions’ Home Health Workshop results in a value-based directional roadmap with defined priorities to move towards a modernized home health experience for patients and providers.

Hitachi Solutions’ Home Health solution leverages Microsoft Dynamics 365 Field Service embedded in Microsoft Cloud for Healthcare, Dynamics 365 Customer Service, and Microsoft Power Apps to deliver intuitive patient portals and mobile experiences for scheduling, optimized routing capabilities for practitioners, and more, to enable best in class experiences for patients and providers. This Home Health Workshop for Microsoft Cloud for Healthcare provides customers with a value-based directional roadmap and an implementation plan with defined priorities to move towards a modernized home health experience for patients and providers. The roadmap will be delivered following a series of exercises led by Hitachi Solutions over the course of the workshop. During these exercises, related business processes, technology requirements, interdependencies, and priorities to achieve optimized care coordination and personalized care will be discussed.
